Welcome to Our GP Resource Hub for Mental Health Referrals

To ensure a smooth and timely intake process, please include the following details in your mental health referral letter:

• Referring GP’s name, provider number, and clinic or medical practice details
• Date of referral and GP’s signature
• Number of sessions being referred (typically 6 sessions initially under a Mental Health Care Plan, with 4 additional sessions upon review)
• Reason for referral, including presenting mental health symptoms and/or diagnosis 
• A statement confirming that a Mental Health Care Plan (MHCP) has been prepared (please attach the MHCP if available) 
• The patient’s full name, date of birth, and up-to-date contact information
   

Providing complete and accurate information helps us deliver prompt and effective psychological assessment and treatment.

We welcome mental health referrals via fax or email—both methods are accepted and secure.
