A Guide to Referring to Affinity Therapy

Welcome to your dedicated resource for streamlining psychology referrals. This page is designed to assist GPs in identifying mental health needs, understanding available psychological services, and making effective, timely referrals. We are here to help you connect patients with the right support.

Referral Requirements

To ensure a smooth and timely intake process, please include the following details in your mental health referral letter:

  • Referring GP’s name, provider number, and clinic or medical practice details
  • Date of referral and GP’s signature
  • Number of sessions being referred (typically 6 sessions initially under a Mental Health Care Plan, with 4 additional sessions upon review)
  • Reason for referral, including presenting mental health symptoms and/or diagnosis
  • A statement confirming that a Mental Health Care Plan (MHCP) has been prepared (please attach the MHCP if available)
  • The patient’s full name, date of birth, and up-to-date contact information

How to Send Referrals to Affinity Therapy

We welcome mental health referrals via fax or emailβ€”both methods are accepted and secure.

  • Fax Referrals: Please include the appropriate area code to ensure successful transmission.

At Affinity Therapy, we strive to make the referral process for mental health services simple and efficient for GPs, allied health professionals, and mental health care providers.

Client Intake Process for Mental Health Services

We will receive the client’s Mental Health Care Plan (MHCP) and referral through fax, email, or directly from the client. Once we receive the referral, our intake team will contact your patient within 24 hours to begin the mental health intake process. Our objective is to schedule their initial therapy or counseling appointment within 2 to 4 weeks. After the client’s first mental health appointment, we will send you an update confirming their engagement with our services. Our streamlined intake and referral process ensures timely access to quality mental health care.
